Muang Thai Smile Health Check Voucher
|
|
| For the customer with following characteristics : |
| 1. Only for Prestige customer (with at least a total annual premium payment for every policy of 250,000 baht) and the customer would receive the health check voucher on the premium payment due date from the renewal of year 2 onward. |
| 2. 2. Policy with eligibility in the project has to be an ordinary life policy or PA policy only, excluding industrial, group, and policy written under the Aue Ar Thorn (generosity) project. |
|
| Remark: the customer would not be eligible, in case they hold only PA policy without ordinary life insurance as main policy. |
|
| 3. Every policy shall not recently have an outstanding APL balance. |
| 4. The policy has to be during the premium payment period. |
|
5. In case of monthly, quarterly, or semi-annual mode of premium payment, the Company would annualize the premium before calculating the eligibility for this project.

Remark : in case the insured is over 60 years old, the eligible options include program 3 or 4, and from January 1, 2009, the hospital in the abovementioned list with * would no longer provide the health check description 11 and 12.

| The condition on the eligibility of health check voucher |
| 1. To validate the right to receive this privilege each year, the customer has to pay a total premium accumulated from every policy of 250,000 baht or more for such particular year and the premium has to be paid within 90 days from the premium due date of policy year 2 onward. |
| 2. In case the customer pay the premium after the due date (as required in 1), or has APL status, or stop paying premium due to any cause, the Company would reserve the right by stop issuing the health check voucher for such year. |
| 3. The Company would deliver the voucher within 30 days from the date of premium receipt for the renewal of policy year 2 onward, where the Company would consider the payment of the first life policy as the receipt date and the customer has to be a Muang Thai Smile Club member only. |
| 4. The health check voucher of the following years would be issued 12 months after the first voucher was issued. |

5. The voucher would valid for 6 months from the date of issue (the voucher has indicated the validity period).
|
| Privilege of the health check voucher |
| 1. Free indicated Smile Health check program, if exercises within the validity period stated on the voucher. |
| 2. The customer could take another two persons to take the same health check program on the same date, time and hospital; and pay a special rate (for more information, please contact the hospital). |
|
3. The voucher has to be exercised prior to the end of its validity period stated on the voucher

The above information is merely about the privilege under “Muang Thai Smile Health Check” program, the Company reserves the right to change the condition and privilege in the future as it deems fit.
