Conditions for Smile Exclusive Health Checkup Program
1. Please make a reservation at least 3 days in advance before receiving the health checkup service at the participating hospitals.
2. Please refrain from drinking and eating at least 8-12 hours before receiving the service.
3. Please present the ID card and the gift voucher given by the Company to the staff at the participating hospitals.
4. Service conditions are as specified by the participating hospitals.
Conditions for Smile Exclusive Dental Services Program
1. Please make a reservation at least 3 days in advance before receiving service at the participating hospitals or dental clinics.
2. Please present the ID card and the gift voucher given by the Company to the staff at the participating hospitals or dental clinics.
3. All types of dental services are available under a limit of 10,000 Baht.
Conditions for Anti-Aging Program
1. Please make a reservation at least 3 days in advance before receiving the health checkup service at the participating hospitals.
2. Please present the ID card and the gift voucher given by the Company to the staff at the participating hospitals.
3. A member is entitled to only 1 Anti-Aging Program.
1. A Muang Thai Smile Club member in The Ultimate tier is entitled to receive either Smile Exclusive Health Checkup Program or Smile Exclusive Dental Services Program or Anti-Aging Program.
2. This privilege can be transferred to a family member. They must be the member’s legal husband, wife, child, father, mother, or the premium payer only.
3. 1 gift voucher for the health checkup service is for 1 person only.
4. This privilege can be used once a year. The service at the participating hospitals or dental clinics is available until 31 December 2024 only.
5. In case a member receives another service which is not the specified service, the member is responsible for the incurred expenses.
6. Service conditions are as specified by the participating hospitals and dental clinics.
7. Muang Thai Smile Club under Muang Thai Life Assurance PCL is only the privilege provider. The Company is not involved in providing the service.
8. The Company reserves the right to amend, revise, add or terminate conditions without prior notice.
