You can apply for this insurance at the headquarters or MTL branches nationwide.

Premium payment option via credit card is not available for this product.
Yes, you can. However, the sum insured will be divided by the number of borrowers in an equal proportion.
You will receive life and total and permanent disability coverage.
If you apply for the insurance with the sum insured of not over 4 MB, health checkup is not required. However, you are required to declare health information in the insurance application form and the health declaration is considered as a part of underwriting or insurance claim payment consideration. If the insured exceeds 4 MB, health checkup is required according to the criteria specified by the Company. Underwriting is subject to the Company's rules.
In case the sum insured is not over 4 MB, use the life insurance application form for life insurance policy, loan protection, single premium payment, ordinary insurance (no health checkup). In case the sum insured is over 4 MB, use the life insurance application form for life insurance policy, loan protection, single premium payment, ordinary insurance, with detailed health questions.
The insured can choose the effective date which is the same date as the loan contract formulation date. If the effective date specified in the application form does not match the date of loan receipt, the effective date can be changed and the sum insured can be reduced to be equivalent to or close to the loan amount within 30 days from the date of receiving the policy.
The insured can cancel the policy under the 2 following cases.
1. In case the insured has not yet entered into the loan contract, he/she can cancel this policy by returning the policy to the Company within 30 days from the date he/she receives the policy. The Company will return the insured the remaining premium after deducting actual health checkup expense and the Company's expense at the rate of 500 Baht per insurance policy.
2. In case the insured has already entered into the loan contract, he/she can cancel this policy by requesting for policy surrender. In case the debt has not fully repaid, the primary beneficiary's consent in writing is required (lender).
-For coverage period less than 10 years : Premium of this insurance contract is partially eligible for personal income tax deduction according to the Notification of the Director-General of the Revenue Department on Income Tax No.383.
- For coverage period from 10 years or over : Premium of this insurance contract is eligible for personal income tax deduction according to the Notification of the Director-General of the Revenue Department on Income Tax No.172 and No.383.
The insured will receive the membership card. Accumulated point calculation is subject to the Company's rules.

In case of death
1.1 Policy with contract period of less than 2 years
- Original copy of policy or loss report document in case of loss of the original policy, copy of death certificate, copy of house registration, copy of ID card of the insured and the primary beneficiary and all secondary beneficiaries, declaration of beneficiary (can be received at the Company), declaration of physician together with medical certificate (can be received at the Company), power of attorney according to the Company's form, and original copy of policy (In case of document loss, submit the police report instead)
1.2 In case the policy with contract period of more than 2 years and the insured passes away due to illness, required documents are as follows:
- Original copy of policy or loss report document in case of loss of the original policy, copy of death certificate, copy of house registration, copy of ID card of the insured and the primary beneficiary and all secondary beneficiaries, declaration of beneficiary (can be received at the Company), and declaration of physician together with medical certificate (can be received at the Company)
- In case the insured has an original copy of critical illness policy (In case of document loss, submit the police report instead)
1.3 In case of death due to accident or being murdered, prepare documents in addition to Item 1.1 and 1.2 as follows:
- Copy of police daily report
- Copy of post-mortem examination report
- Autopsy report (in case of autopsy)
In case of total and permanent disability
- Copy of house registration and copy of ID card of the insured and beneficiaries
- Disability form from the attending physician (can be received at the Company)
- Medical treatment record (if any)
In case of a request of claim for total and permanent disability, submit the above-mentioned evidence within 30 days from the date when the disability occurs. Failure to request the claim within the specified period does not invalidate the right to request for claims if the evidence that shows reasonable cause that the claim cannot be requested within the specified period, and the claim is made as soon as possible is shown.
