All about Claim Steps

Accident claim service in case of advance payment

Do you know you can claim for the medical expenses from the Company after you had an accident and were hospitalized (according to the coverage conditions in the insurance policy)?


Step
1: Prepare documents 

For Outpatients (OPD) and Inpatients (IPD) in case of Medical Expenses

1.Injury/Illness Claim Form               

  • For the insured – The insured/the guardian must completely fill in and sign the form.
  • For the attending physician’s reports - The physician must fill in details and sign as well as affixing medical center’s seal.

2.X-ray diagnostic test results (in case of broken bone or fracture)       
3.Copy of medical expense receipt and list of medical expenses in detail or copy of CUSTOM-IN-PATIEN summary
(in case of hospitalization as inpatient)

For accident claim according to personal accident insurance contract

1.Injury/Illness Claim Form                               

  • For the insured – The insured/the guardian must completely fill in and sign the form.
  • For the attending physician’s reports - The physician must fill in details and sign as well as affixing medical center’s seal.

2.X-ray diagnostic test results (in case of broken bone or fracture)       
3.Original copy of medical expense receipt and list of medical expense in detail or CUSTOM-IN-PATIENT summary
(in case of claim under Personal Accident Contract)

Remarks:
1.In case of signing by fingerprint, signatures of 2 witnesses must be completely provided.
2. In case of a minor (less than 10 years old), a father/mother/legal guardian is required to sign and specify the relationship.
3. In case of a minor (over 10 years old but less than 20 years old), a father/mother/legal guardian is required to sign together with the minor and specify the relationship.
4. In case the insured is a minor (less than 20 years old), the following additional documents of a father/mother/legal guardian are required to be submitted
    1. copy of house registration
    2. copy of ID card.

 
Step 2 : Collect documents and send them via postal mail to

Muang Thai Life Assurance Public Company Limited, 250 Rachadaphisek Road, Huaykwang, Bangkok 10310, or submit the documents by yourself at any Muang Thai Life Assurance’s Customer Service Center nationwide               

Processing Period : Around 3 business days after the Company receives complete and correct documents and information

Remark: Conditions are as specified by Muang Thai Life Assurance PCL.
