Group Health and Accident Claim When Advance Payments from Customers Are Required
Step 1 Prepare
outpatient (OPD) and inpatient (IPD) documents.
1. Medical certificate or claim form in case of injury/illness
2. Original medical expense receipt and medical expense summary
Step 2 Submit the documents to the human resources department of your company
or throughMTL Click Application.
Remark:
The Company will complete the claim consideration within a period of 15 days from the date that the Company receives complete documents, or within a period of 90 days in case additional documents are required.
In case a request to the Company to reconsider group health and accident claim result
Steps
1. Record the request of claim result reconsideration.
2. Additional attending physician’s opinion or total medical records, as the case may be
3. Submit the documents to the human resources department of your company.
Remark:
The Company will complete the claim reconsideration within a period of 30 days from the date that the Company receives the request or complete additional documents.
