- Collection by Proxy: Please present a copy of the Muang Thai Smile Club membership card or a copy of the member's national ID card (with the religion section blacked out). The document must be signed to certify it as a true copy and must include the following written statement: ""I, (Member's Name), hereby authorize (Representative's Name) to collect ...... ticket(s) on my behalf."
