Autopay with Credit Card

Payment is accepted via credit cards issued by all banks with the following logos:

(Applicable only to insurance plans that the Company permits to be paid by credit card, in accordance with the Company’s terms and conditions.)

How to Apply for Autopay with Credit Card

1. Online Application

  1. Apply via the MTL Click Application Click ›
  2. Apply via Standalone

2. Paper Form Application

  1. Complete the “Consent Form for Insurance Premium Payment via Credit Card / Letter of Consent Authorizing the Bank to Debit a Deposit Account, and Request Form for Receiving Policy Benefits and Other Payments via Bank Account.”
  2. Submit the form (1 original set) together with 1 copy of the credit card to the Company.
  3. Submit the documents at any Muang Thai Life Assurance Customer Service Center nationwide, or send the documents to:
    Muang Thai Life Assurance Public Company Limited, 250 Ratchadaphisek Road, Huai Khwang District, Bangkok 10310.
